Editor’s Choice Awards

The Editor’s Choice Awards were developed to give credit to those devices which are outstanding in their treatment categories. These awards are based on survey results by Board Certified Dermatologists and Plastic Surgeons who are renowned experts in laser and light-based devices. The award concept, categories and accompanying comments were developed and written respectively by David M. Cauger of Boston Aesthetics, LLC. Below you will see the ATnT Award and Honorable Mention for each Treatment Category.  Aesthetic Trends & Technologies assisted in gathering this information and is no way responsible for the outcome of any individual or entity whether positive or negative if they choose to purchase one of the lasers noted in these awards. The objective of these awards is to recognize excellence as well as to provide an avenue of understanding of what devices are available and suggested by your peers.

BEST LASER FOR ACNE

Smoothbeam by Candela Corporation

This 1450nm Diode selectively deposits heat in the upper papillary dermis where the sebaceous glands are located. This approach differs fundamentally from shorter wavelength lasers designed only to inhibit bacteria. The Smoothbeam approach is to affect the gland thermally so it produces less oil which bacteria live on. In theory, this would permanently reduce acne lesions. Candela received FDA clearance for back acne last year and facial acne last month.

ClearLight by Lumenis Inc.

The ClearLight operates in the 405nm to 420nm range with a massive 30 x 30cm spot. The wavelengths induce the proliferation of endogenic porphyrins, which attack and destroy P.acnes bacteria. Greater numbers of dermatologists are finding more of their patients are resistant to all antibiotic treatments. The reasons are uncertain, but the ClearLight is a great alternative in these and other cases.

BEST NON-ABLATIVE LASER FOR

WRINKLE REDUCTION

Smoothbeam by Candela Corporation

This 1450nm Diode selectively deposits heat in the upper papillary dermis where an immune response results in the synthesis of new collagen. It is important to note that collagen synthesis below the upper papillary dermis may not yield a visible result. Good results have been noted for 2 years with mild to moderate rhytides.

Profile 1319nm by Sciton

The Sciton Profile can be ordered in many configurations to suit one’s needs. For non-ablative the suggestion is to configure it with the 1319 Nd:YAG. The physician may add the long pulsed 1064nm and 2940nm wavelengths to this versatile platform as well.

ATnT Award

BEST LASER RESURFACING: ER:YAG

Profile 2940nm by Sciton

The Sciton Profile can be ordered in many configurations to suit the physician’s needs.
For precise ablation the 2940nm Erbium YAG would be the wavelength of choice. The Er:YAG can achieve up to 50 pulses per second at 0.9 Joules per pulse. It is debatable whether higher rep rates at lower joules is preferable over higher joules and lower rep rates. Nevertheless, the Er:YAG is making a big comeback. Progressive practitioners are using new methods to minimize downtime.
Honorable Mention (Tie) - Focus Medical & Cynosure Inc.

NaturaLase by Focus Medical

One great secret in the industry is the NatruraLase Er:YAG. The output is a hefty 3 Joules per pulse at 8 pulses per second. This device represents a new approach to the Er:YAG which has many advantages at a great price. The unit has fluence corrected multiple spot sizes between 4 to 10mm. No scanner is required with this device, and pain levels are significantly reduced.

CO3 by Cynosure Inc.

Cynosure takes a similar approach to the NaturaLase with 2 Joules per pulse and a max rep rate of 10. Very flexible Er:YAG with a variety of setting options and 3, 5, and 7mm spot sizes.

BEST NON-LASER DEVICE FOR WRINKLE REDUCTION (RF)

ThermaCool by Thermage

This RF device has been of great interest to the industry since it made its formal debut at ASLMS in Atlanta. Clinicians report the “tightening” effect of a CO2 laser without the downtime of the CO2. Reported pain levels range from moderate to extreme.

Aurora by Syneron

The Aurora is a combination RF and IPL device. The synergy between the two modalities produces interesting clinical end points and results. This technology is called ELOS. The Aurora is also FDA cleared for hair removal, and does very well with pigmented lesions.  Many users indicate it is particularly effective at treating melasma in some patients.

ATnT Award

BEST LASER RESURFACING: CO2

UltraPulse Encore by Lumenis Inc. While CO2 procedures have been declining, they still produce the most dramatic results in facial skin rejuvenation. The UltraPulse still reigns supreme in the CO2 category. No Honorable Mention for this category.

ATnT Award

BEST LASER FOR TATTOO REMOVAL

VersaPulse C by Lumenis Inc.

One of the great aesthetic lasers of all time, the VersaPulse C has three Q-switched wavelengths: 532, 755, and 1064. These wavelengths are required to address most of the ink colors with which the practitioner is presented. The VersaPulse C w/ Help G also can shift into long pulse mode and an even greater variety of treatments.

MedLite IV by Continuum Biomedical

Arguably on a par with the VersaPulse C, this Q-Switched frequency doubled YAG uses a variety of Dye packs to address the various ink colors. A Brilliantly designed laser which has respect of all users. More affordable than the VersaPulse C.

BEST IPL DEVICE FOR SUPERFICIAL VASCULAR

Quantum SR by Lumenis Inc.

Lumenis’ predecessor company Shaplan developed the first IPL device and put it on the aesthetic map. This is a tough category as many IPL’s can effectively address superficial vascular. Unlike other IPL units, the Quantum has integrated epidermal cooling.

Aurora by Syneron

The Aurora is a combination RF and IPL device. The synergy between the two modalities produces interesting clinical end points and results.

BEST IPL DEVICE FOR AESTHETIC

PIGMENTATION PROBLEMS

BEST LASER FOR STRAIE (WHITE)

ReLume by Lumenis Inc.

Utilizing the 290-320nm spectrum with a Mercury vapor arc lamp, the ReLume device received FDA clearance for treating leukoderma. This device is can be used for hypopigmenation as a result of other laser treatments, stretch marks, post surgical scars, post tattoo laser treatments and a host of other conditions where loss of skin pigment has occurred.

XTRAC by PhotoMedex

On par with ReLume, this 308nm Excimer was originally designed to treat mild to moderate psoriasis where it has proven a valuable tool for the disorder. It is also a narrow ban UV laser and will provide the same range of treatment options as ReLume. As with ReLume, clinical outcomes can range from spectacular to poor. This depends on what one is treating and the patient.  Narrow band UV lasers can succeed where all other treatment options are impractical, inferior, or pose unnecessary risks to the patient.

BEST OVER-ALL VALUE FOR AN IPL DEVICE

EsteLux by Palomar Medical

This portable and powerful IPL is setting a new standard in IPL technology. With six hand pieces to choose from and a list price of $42,900, this was a clear winner.

SkinStation by Radiancy

The Skin Station is also portable and comes with a variety of hand pieces. It is not as powerful as the Estelux, but at a list price of only $25,990 the SkinStation is a fantastic value.

MOST AFFORDABLE IPL DEVICE

ATnT Award

SpaTouch by Radiancy

The SpaTouch list is $14,900. While not the most physically appealing product, it does indeed work for hair reduction and pigmented lesions on lighter skin types. Interestingly enough, the Spa Touch has the largest installed base of any light based aesthetic device in the world.

ATnT Award

SkinStation by Radiancy

The Skin Station comes in significantly lower at $25,990 than Palomar’s NeoLux ($32,900). Radiancy is still the undisputed leader in value IPL devices.

MOST DIVERSE AESTHETIC LASER

XEO by Altus Medical

Altus Medical continues its mastery of the long pulsed YAG with the XEO. The XEO incorporates effective hair removal vascular, non-ablative and now pigmented lesions with an integrated 600-800nm IPL device. Too many options to mention. Massive power and extremely fast rep rates. This laser is truly the best aesthetic long pulsed YAG every built. The only downside is the price tag at $145,000.
